Mr Aari Vanov, Age – 69 years

Dear Ms. Jolly

I am writing to refer Mr. Vanov, who has been under my care for six weeks. At the time of admission, he was unable to perform his daily activities and demonstrated aggressive behavior. He has calmed down since then but requires regular home visits by a music therapist to reduce his aggression.

Initially, he was aggressive and hit his co stayer due to which he had to be transferred to a single room. Mr. Vanov rejected care by female staff; therefore, a male nurse was assigned for his care and he has helped him reduce / to reduce his aggression and commenced a healthy, low salt diet for him.

In the second week after admission, he demonstrated verbal aggression and shouted at fellow residents. He is now calmer and has started participating in the weekly singalong.  He has also given consent to commence music therapy.

It would be appreciated if you could make regular visits for him as it will help to improve his overall well being.
