Re: Miss Catherine Brown, 13 years of age
I am writing to refer a 13-year-old girl who has numerous symptoms / signs and symptoms indicative of Takayasu arteritis. Your close collaboration with other specialists to address her problems would be highly appreciated.
Today, Miss Brown with her mother presented to my practice to discuss the health concerns of her child. The mother has been worrying over the frequent episodes of epistaxis and generalized unrelenting headache. A previous physical examination had revealed a discrepancy in her arm BP readings. While the right arm had a normal reading of 120/80, the left arm had a considerable higher reading at 170/120 mmHg. Regarding blood test results, mild anemia and slightly elevated ESR were noticed. Cardiomegaly was evidenced from both chest X-ray and EKG. The most striking features were the dilated aortic arch with narrowing of its branches and the concomitant presence of suprarenal aortic stenosis.
Those findings and a possibility of being diagnosed with Takayasu arteritis were explained in detail. Angiography and stenting of abdominal aorta was concurrently planned. Two strength of amlodipine 5 and 10mg, 100mg atenolol, 6mg prazosin and 50mg hydrochlorothiazide were prescribed. Additionally, a weekly dosage of intravenous 5mg methotrexate with folinic acid has been started.
Miss Brown was diagnosed with hypertension early in her life in 2013 for which amlodipine and atenolol were commenced.
