Re: Mr. James Smith, D.O.B. October 26th, 2004

I am writing to refer a high school student who is recovering from pneumonia associated with cystic fibrosis. Your assessment of whether bilateral lung transplant might be an option for him will be highly appreciated.

Today, Mr. Smith is due to be discharged after a successful treatment of severe pneumonia and chronic sinusitis related to cystic fibrosis. He has been informed of an increased risk and frequency of subsequent lung infections. Therefore, an alternative method of education like homeschooling was discussed and chest physical therapy with dietary modification was recommended / advised.

Mr. Smith was admitted on February 2nd, 2019 with shortness of breath, fever and hemoptysis. The significant findings were tachypnea with RR at 30/min., oxygen desaturation at 80%, low-pitched rhonchi and bilateral inferior lung field crepitation indicative of lower respiratory tract infection. Apart from oxygen supplementation, his initial treatment plan included the administration of amoxicillin/salbactam which were replaced by azithromycin and ciprofloxacin the following day. The result of imaging studies confirmed the diagnosis and chronic sinusitis was also noted.

Mr. Smith has a history of multiple episodes of chronic sinus infection and focal epilepsy. He was diagnosed with cystic fibrosis in 2006. He has no significant family history and no allergies. His current medications are 300mg phenytoin daily, dornase alfa and pancreatic enzymes. Please note that Mr. Smith is overweight with BMI at 28.5 due to sedentary lifestyle.
