Dear Dr. Clarke,

Re: Mrs. Maria Santini, D.O.B. January 8th, 1948

I am writing to refer a 67-year-old widow who suffers from deteriorating bilateral knee osteoarthritis and Baker’s cyst on the right one. Your assessment and management of her knee condition by administrating an intra-articular steroid injection would be highly appreciated.

Today, Mrs. Santini visited my practice complaining that the pain in her knee joints had been getting worse over the last six weeks. She also reported having a soft mass behind her right knee which limited her joint movement and minimal to moderate joint pain for the last 4 weeks. All of these have affected her day to day activities, her attitude and her mood. Upon examination, it was found that she was overweight. Also, her joint produced a cracking sound when it was in motion.

 She was diagnosed with moderate osteoarthritis with Baker’s cyst formation in the right knee which was confirmed by Knee MRI. Her treatment plan includes a physiotherapist referral and assessment of home living quality by a community nurse.

Mrs. Santini has a history of knee osteoarthritis onset in 2003. She underwent a lumbar laminectomy and a bilateral hip replacement in 2006 and 2010, respectively. She also has hypertension, hyperlipidemia and was diagnosed with paroxysmal atrial fibrillation in 2006. Her current medications are glucosamine, trandolapril, indapramide, simvastatin, flecainide and digoxin. She does not smoke or drink alcohol and has no known allergies. Please note that her mother was diagnosed with and probably died from myocardial infarction.
