Re: Ms Anne Hall, 45 year old patient

I am writing to refer Ms Anne Hall, who is probably suffering from gastro-esophageal reflux with or without stricture. She requires further investigation for a definitive diagnosis.

Ms. Hall arrived here / visited my office earlier today complaining of dysphagia / difficulty swallowing food, especially solids. The onset of her condition was 2 weeks ago, possibility after an upper respiratory tract viral infection. 

She took over the counter Chinese herbal product for the viral infection. The dysphagia is accompanied by epigastric pain radiating to the back at T12 level. She takes aspirin 2-3 times a month. Upon physical examination, no lump was food. She also did not show any signs of anxiety or any remittent course of the viral infection. She was advised to reduce coffee and alcohol intake. Furthermore, she was prescribed Pantoprazole.

Ms. Hall has a medical history of childhood asthma, chickenpox and measles. She sought weight reduction 10 years ago and suffered from a bout of dyspepsia in 2012. She has a significant family history of peptic ulcer and asthma. She is allergic to codeine, dust mites and sulphur dioxide. She is a social drinker and has had a recent increase in coffee consumption.

I would appreciate it if you could assist Ms. Hall with further investigation and a definitive diagnosis.
